A schedule of benefits is a list of the various services covered under a health insurance plan and the associated fees. Policyholders receive them so that they know which services and treatments are covered and which are not. Some treatments and services may require a co-pay, and others may not.

A schedule of benefits would list various preventative services, hospital services, diagnostic services, therapy services, urgent care services, and any other health care service covered. For example, under preventative services, it may list adult physical examination, 1 exam per calendar year; vision exam, 1 every 12 months; and hearing exam, 2 per calendar year, alongside their associated co-pay costs.
